For chronic use. Students can be administered up to 3 doses before you will be requested to supply your child with their own medication.
HIPAA Record Release Form
Medical records cannot be released without signed authorization. This form must be completed and returned to nursing staff. It may take up to 48 hours for requests to be completed during normal business days.
ISBE School Health Requirements
Well Child Exam/School Physical
All Freshmen are required to have a completed physical before registering for school. New incoming students from other schools in Illinois will also be required to have a copy of their freshmen physical.
New incoming students from out of state will be required to submit an Illinois school physical within 30 days of registration.
Immunization Requirements
Freshmen and incoming students will need to have an up-to-date immunization record included with their school physical.
After the age of 16, students will be required to meet meningococcal requirements before starting their senior year of school.
